It's probably not anything you did. If you messed up the code on your end you shouldn't see your own member id number in the display. In theory...
To my knowledge there is not a variable that will directly grab the members name. However the [memberID] part will snatch the Member ID number when the person enters. Then there would be another process on the 3rd party server can would query the UV servers, load that profile page via the member id number, and grab the member name. That's how it's done. There are two things happening to get the user's name.
But, if --for instance-- Utherverse has blocked access to the UV servers by this 3rd party other server (servers have static IP numbers, so easy to do) for some reason, the block would effectively break the tools ability to grab the name.
The same sort of thing would happen if UV's servers happened to be under heavy load when you were loading the region, under heavy enough load they they couldn't respond to the 3rd party server in time to get the name displayed. This would be a Temporary issue. Meaning one time you would see your name, another time you might not.
If I had to hazard a guess, one of the two above would be mine. But it's purely a guess on my part, not knowing any of the specifics. A guess where the symptoms match what would happen if an IP block was slapped on a 3rd party server for any reason.
